There will be an additional fee of $100 if these forms are not sent in by the last payment deadline. Please send them in as soon as possible.
All trips and scheduled ministry are subject to change due to unforeseen circumstances that may arise. If fuel and airline costs drastically increase, JSMI reserves the right to adjust the prices accordingly.
All contributions must be used on a JSMI trip within one year; no refunds will be offered. Please read all Policies & Procedures for more details.
